That more or less answers your second question, the cock head is exposed during sex. Would be pretty pointless, if it wasn't.

I find that it retracts itself. Depending on arousal levels, sometimes it retracts before any penetration or manual pulling back. So yes, the head is generally retracted during penetrative sex. Having said that, I once had a partner who didnt realise that the foreskin should be retracted before putting on a condom so the foreskin stayed over the head and held there by the condom. Generally, my foreskin does not automatically retract when I become erect, although it does retract a little. If I keep it pulled forward as I become erect my pee hole widens. When I pull my foreskin back the pee hole closes. During sex the foreskin moves a little.
