Well, the cum has to get out somehow. For me it usually just flows out during teasing after some denial (usually already one week can make that happen). I wouldn't call that an orgasm, because for an orgasm, even a ruined one, you still go to the edge and a bit over it, and usually there is at least one contraction and a a bit of a decrease in horniness. Even if it is only for a few minutes.
