i think they've got three different lengths of spike the longest of which is not for long term use but more as a punishment. However some models are made with staples as the spikes!!

i think they only get you if ur more than semi hard but like you said its hard to tell

Not that I've ever checked into these, but I think the idea is that the cruel little spikes are pointed out slightly, so if you tried to slide it off (even when soft), they would grip into the flesh.
