It uses a different tone for normal throat fucks and deepthroat holding. I think this person's other work(you can look at the profile on the hypnotube site) is a little fast and generally not really a normal deepthroat/facefucking pace, it's also sissy focused, which isn't my thing, but this newest one seems to be a little more focused on the deepthroat stuff and didn't seem to bother me as much with the sissy talk, but your mileage may vary. I haven't played with it much, but perhaps a graphic equalizer to remove vocals or use a vocal removal function in an audio editor might help. :shrug:
I agree, there isn't too much of this metronome stuff that doesn't try to put a cock image in front of the person playing it and I find it hard to actually look up at a screen when it seems more natural to have a dildo on a wall in front of me or below me so I can face down towards the dildo.

There's also a script called Oral Trainer on Sexscripts ss.deviatenow,com Search for Oral Trainer. I realize this is off the Milovana platform but it's one of my favorites for this because it has different functions for positions(gives random numbers 1-10 for position you move with the dildo in the mouth), has speed runs, and that sort of thing. It seems to want to run faster and faster with each run, so I ended up modifying the script in a text editor so I could set the speed manually and it stays at a speed comfortable for me. It uses audio files for up, down, the numbers, etc so it doesn't rely on needing to see the screen to use it at all. It was one of the first oral based things I came across that I felt really worked well for me. There's also PunishmentChooser where if you manually choose one of the three(I think) "Punishments" focused on deepthroat stuff that seems pretty good. You can also modify the script to do what you'd like as far as how long it does each part of the task to make it into what you want too.
I realize not all of this is quite what you are looking for and you might have already ran into most of this stuff. I'm also looking for stuff that is more along the lines of what you are looking for but it's hard to make this stuff when we all have different preferences, I've tried to make things for myself but always get stuck with how to make it work the way I want it to, but I hope this helps a little.
