is that the only pic.. mybe its just staged.. if not thats pretty freaking scary (and not to mention dangerouslooking) but then again there are some cinease monks that strengthen their ballsacks by hanging heavy weights from them.. some are soppedly able to support their own body weight or more

Yeah it's ok, I should've put a warning in the title myself.

It was a part of a pic set, there was 2 or 3 pics out of the 10 or 12 pics with the balls like that. It looks like they're twisted far too much.

It's a top perspective, the penis is folded once and tied in that position. Also, he's just lying on his back, ass up, so balls are not tied at all.

The tie itself is not very painful. But it can get pretty uncomfortable if there is a lot of pull on the top rope.

The penis is impressively flexible and therefore can look pretty messed up before it even starts to be painful. Reminds me of an interview with Johnny Knoxville of Jackass. He asked the (female) interviewer to ask Steve-O "what time it is". She did. Steve-O turned around, pulled down his pants and turned back with his penis wrapped around his arm like a watch.
