It’s terrible quality as it’s the only image I could find
The ring around the cock is spiked, which made me think it was kalis teeth, but afaik kalis teeth doesn’t have the ring to trap the balls?

Could be a custom made one. On Aliexpress they are kind of trending, they often call them "training chastity devices". They may also have some available with attachable screws/pins.
