Image upload sometimes doesn't work

Sometimes when i try to upload an image, it just doesn't work.

Some images work and some don't. I don't think it has anything to do with resolution, image size, or file type.

I've also tried it on chrome and firefox.

i have an image, but what i couldn't show is that if i mouse over the "image uploaded" area, it brings up a little textbox that says "no file selected"
