Some of the best e-stim pain sections in narrative format IMO, is still in the numerous Webteases that implemented pain in their stories. Like the dungeon in Bryci's or the basement in Unexpected Encounter. I've basically never found a estim video that really had a decent pain section anywhere close to the tension in those.

One thought is that basically any of the videos will be painful if you turn it up high enough, find how much you can take as it turns in to pain for each video, then start each several levels higher, lock yourself in with some self bondage and away you go

I'm in the exact same camp as you and also really love when content suddenly and unexpectedly employs pain, especially in a narrative-supported way. And not just in a "I turned up the intensity/volume hehe" kind of way - the signal itself has to change because there's a world of difference between someone squeezing your cock much harder during a handjob and someone suddenly pulling out a flogger!

I agree with MegaErato that there are some pretty good webteases with pain, but I've found they can be a little hit-or-miss for me and my stereostim box. Some pain sections just aren't nearly intense enough, they just assume that rapid short pulses automatically equals pain, or both. For me, I even tried downloading and editing the GuideMe version of Bryci's Estim Experience to try and make the basements far more intense, but I never could figure out how to do that properly and only created a bunch of glitches. haha
Estim Hack and Estim Journeys both have some pretty good sections, especially if you want an interactive experience (they don't work ideally for self-bondage though). And both Estim Laboratory, and especially Estim Bondage have some extremely intense pain involved, but I'll just say be careful and don't blame me if you fry your dick.

Aside from those and the examples provided by others above, if you're like me, I think you may really enjoy Mistress And The Remote Control Box by phx/Aquarium Parrot, especially if you can track down what I think is the lolol2 edited estim audio file. It doesn't exactly have any torture sections, but I'll just say that, if calibrated right, I can combine that with self-bondage and reliably cum in under 7 minutes. ;) The entire video is really incredible of course, but it's become my go-to file when I just really need a great orgasm without any delays, so you may also enjoy it!
