For me, and I realise that it must vary for everyone, but it's a combination of all three.

You mentioned the foot; yes. The shape of it, the scent. The shoe; yes also. There tends to be something dangerously feminine yet powerful about the shape of a high heel for instance. The act of subjugation; Again yes! Very much so!
