Depends. Usually though, yes, they touch, poke, prod, scrape, dig, or whatever they need to do, being thorough to get the right answers. Maybe not quite how you think tho. It can be a little embarrassing and humbling the first few times. I recommend getting a regular doc, though, because they get to know you, learn your body, symptoms, lifestyle, etc., and can usually more easily spot any issues that might arise in your life. You build a rapport over time, which makes it less uncomfortable for future visits.
For me it goes something like this: You go in, talk to the doc about how you are feeling, anything weird, not normal, concerns, & the purpose of the physical, if any (a regular checkup, having unusual symptons or occurances, not feeling well or different than normal, turning a certain age, taking on a new carreer or hobby, etc...). I strip to my underwear & sit on their table. The doc inspects your head & body, looking at your skin for cancer, moles, concerns, things that might become issues, or things that shouldn't be there on a normal human body. He looks into your mouth, throat, ears, eyes, & nose with special tools. Doesn't hurt. He listens to your breathing with his cold stethiscope. You breathe deeply. The first several times visiting, he asked about family history & medical issues that might be inherited or more likeky to happen to you if they are common in your family. He tests reflexes & movements of your joints. You stand while that continues. You then bend over so he can look at your spine. Then he says "drop 'em", so down go your underwear. He puts on rubber gloves, pokes around your nuts for a few seconds, asks you to turn your head & cough, looking for a possible hernia... one side, then the other. Back to the table. You get on all fours in a position that your ass is wide open while he lubes his gloved finger. He inserts one into your ass, feels around for abnormalities, & gets a stool sample... maybe like 20-30 seconds depending on what he feels/finds. Then he leaves while telling you to get cleaned up & dressed.
When I had my flight physical, he did a more detailed look/test in my eyes & ears. For my scuba physical, he looked closer at my ears, head, and breathing/lungs. During both of those he explained what he was doing & why, possible concerns in having these hobbies, & when he wasn't asking questions, I was asking him questions. Usually 30-45 minutes total... That's my experience. It's very professional. No worries. Pretty simple. Hope it helps. Don't wait 'til it's 'too late'. They can spot things and catch a lot before it becomes an issue.
You might wanna fake something 'down there'... or even 'up there' if the doc is cute & female!
