Occasionally, I notice messages I send are in my outbox. More than likely your pm did send but didn't update to your sent folder. Or maybe it has to be read first by the recipient to be considered sent. Either way if it is in your outbox, I am sure it worked on your end

Thank You... but no. I just replied to a PM and now I have 2 stuck in the OUTBOX..... This is the absolute DUMBEST PM setup i've ever seen! My ctrl panel says 2 in outbox and neither in SENT. Thank You for trying though .....

Princess Raquel's second thought is right on. A message in the 'Outbox' folders means it has not yet been received by the recipient, while a message in the 'Sent' folder means it has been received by the recipient.
A quick search for 'outbox' in the forum reveals several other posts answering your question:
Posts:
http://www.milovana.com/forum/viewtopic ... box#p10554 (Seraphox: "Always been this way. Outbox simply means that the recipient hasn't read the PM yet. If you send a PM the other user will get it within a few milliseconds, but obviously it can take a while until they login to read it.")
http://www.milovana.com/forum/viewtopic ... box#p13118 (Evals:"Delivery is triggered the moment it's opened by the recipient. It's nice as you can edit the PM if you decide you want to change a bit even after you send it, though the person reading it will see 'x edits have been made.'")
http://www.milovana.com/forum/viewtopic ... box#p10547 (Me: "'Outbox' means they haven't been received by the recipient yet. Don't worry, it's not like your email program -- if it's in your outbox, you've done all you need to do.")

mfire is totally correct.
Untill the reciever opens his / her mailbox, the message you sent stays in your outbox.
And as long as it is in your outbox, you can still alter it.
I have edited some of my mails in the past, placed aditional information below, instead of writing a new message.

OK... Thank you all very much. That's like the only system where you send and it doesn't aknowledge that you've sent. This thing should rename the sent file to a ... DUH receive file. There is no sent file. But really thank you all for your help.

The "system" is very logically and there is no reason to change it. Almost every forum in the internet has this system. Not only phpBB. And phpBB is one of the most used forum softwares.
Maybe we should rename it so really everybody understands it.
For example in: "Sent messages" and "Received messages".
But I bet in your eMail program it is called "Outbox" too

The most eMail providers use similar names.

I think the confusion comes from that Outlook has an "Outbox" which holds the mails that haven't been delivered yet, as in, they are still locally on your computer until Outlook can contact the mail server to send it off to the recipient (for example if you're on dial-up you can put several mails in your outbox and they'll actually be sent once you get online).
So in Outlook a mail in the "Outbox" cannot be read by anyone else, but once it's on the way it gets moved to "sent". And since Outlook holds quite a large chunk of the email market I guess that's why people gets confused. They keep looking for that "Send and Recieve" button.

I will say, I was also baffled by the 'Outbox' system here. I am no Internet forum expert, but I also thought my PM's were not getting delivered.

There's an interesting discussion of this issue in the official phpBB forums:
PM Outbox/Sent Suggestion
What it boils down to is that calling it Outbox isn't quite right (because the recipient can already get it), but calling it Sent isn't really quite right either (because you can still edit or delete it). PMs are similar to email, but there are differences, so it's hard to use the same terminology.
