1. does it do permanent damage, it is extremely unlikely that toothpaste would do any permanent damage, obviously safety issues such as bleeding or such, just call for common sense safety and hopefully, you would not play when previously injured..by whatever..
2. does it kill sperm? probably yes... however sperm die anyway after 3 days in your balls and are expelled when you pee. Would it lower you sperm count? perhaps that day you might be a little lower but you probably won't notice unless you are going to a sperm clinic.

I highly doubt it would kill sperm. I say that because the perceived temperature difference is caused by the menthol (or a similar ingredient) stimulating nerve receptors. There's no real heating or cooling.

Do you really think people would use a toothpaste that could cause a chemical burn to your skin or worse an allergic reaction? This is stuff that goes into your mouth 2-3 times daily, if there was even a mild reaction of any kind that's where it's most likely to happen.

Well, for allergy - there's an allergy to pretty much ANYTHING. Besides, mouth is actually more resistant to chemicals than some of the other areas of the body.
