Medium POI took awhile to get used to. I tried the large POI a few times, but like you said, it's tough to even get in the cage with them. They hurt from the onset... serious pain, and that's when fully soft. I had to take them out after 5-10 minutes. I could barely move while wearing them, couldn't walk. I cannot imagine when an eretion is attempted/happening. I'm pretty sure I'd be bleeding from those large POI. Mediums already bruise me... but I actually learned to like it. Maybe one day I'll have to do better for Miss Jay and try the large again if She wishes.
