Doesn’t the codex only show enemies that you’ve faced and what area they’re in? I haven’t ever seen raven

Love the game! Unfortunately for some reason I cannot get the final torn map piece no matter what I do. I have all the area maps, I've faced all bosses and beat them, but no other special event spawns for the final piece. I have encountered the ghost in the Dark Woods, I'm at a loss.

Edit: Just beat the Forest encounter with the 2 girls, they did not drop me a Piece of Map.

Edit #2: Just beat the Lakeside encounter with the T-Girl, did not drop me a Piece of Map.

Spoiler: show: I'm going to assume you haven't gone and fought the girl the ghost gave you a vision of? Defeat her and you'll gain access to a boss in the Dark Woods.

Hey Donny ! The Forest and Grove share the same map piece, the same goes for Lakeside and Deep Forest.
As @spaisin said, you're probably missing the Dark Woods map, you can read more about it in the game's FAQ here

Triggered a bad ending I had not seen before, but not sure how. Failed an anal check at the stockade, suddenly I am a collared slave? Seems a bit harsh on a single failed unavoidable (random) check?

Hi ! To trigger this bad ending, three things must happen:

Spoiler: show: 1) Everytime you visit the City a random event may occur (this is indeed completely random). The chance of this specific event being triggered is 6.25%

2) You must fail the Anal/Blowjob:10 Check. The game will roll for whatever is your highest addiction

3) The addiction rolled on the last step must be above 25

If all these requirements are met the Bad Ending will be triggered

Spoiler: show: If only steps 1 and 2 are activated, the Hero will receive an addiction gain and the game will continue

Spoiler: show: To defeat Chloe you must raise her Lust to 100.
You can do this by choosing the "Fuck her ____" or "Endure" action. The amount of Addiction and Corruption you have will influence how much Chloe's Lust goes up.

I believe that if you are really in a pinch, you might aswell try your luck and visit the City anyways, as for the chance of that specific event + bad roll happening is relatively low.
But yeah, that may be a good rule of thumb
